To reproduce (in tiki 3.3 without ajax, and in tiki 4.0 regardless of ajax setting):
1. create a sheet2. click insert column (or row)
3. fails with error msg "A SheetId? is required."
In Tiki 3.3, this misbehavior dissappears when you enable ajax.
In tiki 4.0, no way to make it work.It used to work in tiki 2.x, afaik...
- Solution
Fixed in trunk (around r24117) by jonnyb.
and backported to proposals/(3.x and proposals/4.x by xavi.Hopefully included in next stable releases.

Workaround for the time being:
Create the sheet in OOo Calc, export it as csv, and import it in tiki sheet.- Importance
