History: TOC

We have 3 approches for Table of Contents (ToC)
* ((Maketoc))
** It is not a real plugin like the others (there is no help)
** There are issues which are hard to fix in current implementation, such as ((maketoc_encoding))
** If we keep the syntax, it likely should be moved to a JavaScript
* ((doc:pluginTOC)) for ((Structure))
* ((Auto TOC))


Let's rationalize this for Tiki13 / Tiki14 in a modern and future-proof way, along with ((Menu Revamp)) and possibly some work on the ((Structure)) feature.
