We have 3 approches for Table of Contents (ToC)

• Maketoc
  • It is not a real plugin like the others (there is no help)
  • There are issues which are hard to fix in current implementation, such as maketoc_encoding
  • If we keep the syntax, it likely should be moved to a JavaScript
• pluginTOC for Structure
• Auto TOC

Let's rationalize this for Tiki13 / Tiki14 in a modern and future-proof way, along with Menu Revamp and possibly some work on the Structure feature.
